Welcome to SCOTT M DAVIS MD LLC Practice
My medical practice is currently set up now as telehealth-only. At this time, there is no option or need for patients to travel to visit an office. All visits, both initial assessment visit and followup visits are securely conducted through Zoom. Instructions will be given to the patient to download the Zoom app to either their phone or desktop computer, which is required for Zoom connections. In unusual circumstances due to unforeseen connection problems with Zoom or other technical issues, the visit will be conducted by phone-to- phone between myself and the patient. Either telehealth face-to-face interaction or phone call is required for the visit.
When the visit is booked, the appointment can be cancelled or rescheduled if need be. However, once the visit is scheduled and then conducted, it is both non-cancellable and non-refundable.
Payments for services are payable by valid credit or debit card and paid ONLY through the invoice which is emailed to the patient prior to the visit being conducted. Invoicing and payment is made through Stripe card processing services. For the initial assessment visit that has been scheduled, the initial fee of 237.95 is due prior to the visit being conducted with Dr Davis. This is due to the significant time of 45- 60 minutes allotted for these initial visits.
Monthly followup visits with patient are required, and charged at 237.95 for each monthly visit--payable on before the followup visit. Provided there is no "past due" invoice that has not been paid, patients may elect to take up to four weeks to pay invoice before becoming "past due". If there is a past due invoice, prescriptions will not be sent to the pharmacy until "past due" invoice has been paid in full.
